Sherlock Holmes (Nicol Williamson) is living in a paranoid world of cocaine addiction, secluding himself from the outside world. He is harassing Professor Moriarty (Laurence Olivier), his former mathematics tutor; he is convinced that Moriarty is the incarnation of evil.

With the help of Holmes's brother Mycroft (Charles Gray), Dr. Watson (Robert Duvall) hatches and executes a plot to get Sherlock to Vienna for treatment by the famous Dr. Sigmund Freud (Alan Arkin). The treatment is successful and Holmes, recuperated and refreshed, is ready to return the favor by offering to help another of Freud's patients.

The fraulein Lola Devereux (Vanessa Redgrave) is a beautiful and famous mezzo-soprano who recently escaped bunch of mysterious captors who had abducted her. Both Holmes and Freud feel personally motivated to find the perpetrators of this crime and join forces in order to solve this mystery.
